Important Forms.

Please click the below forms to submit them.

Visiting the School

Parents are always welcome at MS104. You must have an ID to sign in at the security desk to gain admission to the school. In order to make the visit more productive, parents are required to make an appointment with the teacher or administrator with whom they would like to meet. Parents may call the Main Office, Parent Coordinator or send a note with their child to make an appointment.

Attendance & Absences

Parents who know that their child will be absent are asked to advise the school in advance. Please contact your child’s teachers via Jupiter and email the Parent Coordinator at BRivera@schools.nyc.gov

Upon their return to school, all students who have been absent are required to bring in a written note signed by their parent or guardian. Parents should provide a doctor's note for all medically excused absences.

If a student is absent without notifying the school for two consecutive days, the homeroom teacher will call the home. Any student who is excessively absent within a school year will be reported to the Attendance Monitor.

Early Dismissal

If you need to dismiss your child early, an authorized adult on the emergency contact card must meet and sign out your student from the Main Office.

For attendance questions, please contact Bonnie Geller-Doyle: bdoyle3@schools.nyc.gov

Lateness

Any student arriving late must enter through the Main Lobby on 20th St., sign in the Late Student Log and pick up a Late Pass. Those who are late will not be allowed to enter class without this pass.

Lunch & Recess

MS104 students may enjoy lunch in the cafeteria or our yard.  Students can use this time to visit teachers during office hours to make-up work or ask questions. Students may also take the "library bus" to visit the library during the lunch/recess period. MS104 does not permit "out lunch".

Cell Phone & Internet-Enabled Electronic Devices

State law prohibits student use of personal internet-enabled electronic devices during the school day. Schools are required to communicate their plans regarding this policy to their school community prior to the start of the school year.

Please click here to access our school's policy for the 2025-26 school year. The policy includes details of our plan for storage of personal internet-enabled devices during the school day (Yondr Pouch System), under what circumstances students can access their devices, exceptions to the policy, replacement fees for lost or damaged pouches, disciplinary interventions for violations of the policy, and information on how to contact your child in the case of an emergency.

Baruch Blast

In order to stay in touch, the MS104 PTA sends out an email newsletter called the Baruch Blast weekly during the school year.  The Baruch Blast is not the only means of communication for the school but is the primary way to help families keep up-to-date with what is happening. To register, click HERE, fill in your personal information, and choose the e-mail list for your child's grade.
